Phase 1: Design (please use the course room as your primary source of information)
QUARTER ONE
· Meet with faculty advisor during the first week of class
· Complete Term Plan ( submit in course room):

· Week 6

A. DNP Project Preparation
i. Meet with practicum site Project Mentor, Faculty Advisor, and yourself to discuss the proposed DNP project.
1. Bring Project Site Approval Form
2. Bring introductory Letter
ii. After the meeting, complete the meeting notes and submit to Faculty Advisor for review
iii. Begin work on:
1. Organizational Readiness Assessment ( submit in course room)
2. Stakeholder Analysis ( submit in course room)
3. SWOT Analysis ( submit in course room)
iv. When Faculty Advisor approves, submit site Meeting Notes and Site Approval Form in Task Stream for full committee approval
B. Question Development
i. Complete question development tool
ii. Meet with DNP Mentor to discuss question development
1. Submit minutes from question development meeting with DNP Project Mentor in the course room
C. Evidence Tables
1. Complete the Individual Evidence Summary Tool (literature matrix) and submit in course room
2. Complete the Synthesis Process and Recommendations Tool and submit in course room
· Note: all of the above must be completed for a satisfactory grade for the first semester of N8702
QUARTER TWO
Phase 2: Plan
· Meet with faculty advisor during the first week of class
· Complete Term Plan: ( submit in course room)
· Week 1
· Week 6
· Week 11
A. Core Design
i. Complete the action planning tool and submit in course room
ii. Complete the Work Breakdown Structure or Gantt chart and submit in course room
iii. Meet with DNP Project Mentor to discuss the Action Plan and Work Breakdown Structure or Gantt chart- submit meeting minutes in the course room
iv. Complete project materials checklist specific to your DNP project
B. Final Action Plan Review Meeting
i. Once your Faculty Advisor has approved the documents above that were submitted in the course room, set up a meeting in Teams with your DNP mentor, any other site stakeholders, your Faculty Advisor and any other stakeholders. During this meeting you will present your final Action Plan using the supplied PP template.
ii. Create a zip file with your presentation, project materials, and the documents listed above and submit to Task Stream for committee review and approval
iii. Complete your Ethics Pledge online
· Note: all of Phase 2 must be completed for a satisfactory grade for the second semester of N8702
Phase 3: Implement
QUARTER THREE
· Meet with faculty advisor during the first week of class
· Complete Term Plan:

A. Action Pan and Progress
i. Meet with DNP Project Mentor and submit meeting notes in the course room
ii. Meet with Faculty Advisor as needed to update and review action plan
Phase 4: Monitor
i. Meet with DNP Project Mentor and submit meeting minutes in the course room
ii. Update the Workbreakdown Structure or Gantt Chart and submit in course room
iii. Update Action Plan and submit in course room
iv. Make any revisions to checklist and project documents and submit in the course room
Phase 5: Close / Evaluation
i. Faculty Advisor, DNP Project Mentor and student meet to evaluate project, submit minutes in course room
ii. Create a draft executive summary and submit in the course room
iii. Submit the Executive Summary checklist in the course room
iv. After Faculty Advisor Approval, submit Executive Summary and Checklist in Task Stream for Form and Style review
v. Prepare and present final close out presentation to Faculty Advisor, DNP Project Mentor, and any other stakeholders (use provided PP template)
Phase 6: Disseminate
i. Submit the Executive Summary in the course room
ii. Submit the Executive Summary checklist in the course room
iii. Submit presentation approval, executive summary and checklist for final committee and CAO review in Task Stream
Final Submission
· Submit the clean copy with front page to [email protected]
· Make sure to delete any consent forms you may have included in the proposal or IRB review stages
